Graduate Mail: Health and Health Behaviors Module

OVERVIEW

The 1993 round of WLS data collection was the first to include items directly
ascertaining health and health behaviors.  In addition to the telephone health
module, this round (2004) replicates most of the key items from the previous
round plus many additional ones, including the SF-12, childhood health, access
to health care, health utilization, quality of life, and future health
projection items.

For brevity and conciseness, items from the 'Health' and 'Health Behaviors'
modules have combined.
