Phone: End of Life Preparations
GEOLPREP End of Life Preparations Module Graduate Instrument 2004, Phone For more information on the End of Life Preparations Module see COR853 in Appendix U. OVERVIEW This module is new in 2004, as is the companion "End of Life -- Death" module. Note that beneficiaries of life insurance are listed in the Assets Module, not in this module. The EOL questions were administered to a randomly selected 70% subsample to reduce the overall length of the survey. However, all of the Wiscville residents were asked these questions, because in the 1990s this area was the site of a large-scale intervention study which encouraged people to make end of life preparations such as living wills. Respondents were considered to be in the Wiscville sample if they lived in the Wiscville area 1) at the time of the 1993 survey, 2) when tracked for 2004 survey, and/or 3) at the time of the 2004 interview. BRIEF VARIABLE DESCRIPTIONS GY001RE: Wiscville Resident Flag (100% of Lacrosse Residents Will Get This Module). GY002RE: Eolflag for Sample Selection (70% of Respondents Will Get This Module, Except for Wiscville Residents, Who are Sampled at 100%, As Mentioned). GY003RE: Has R Made *Any* Preparations for End of Life? (I.E., Positive Response to at Least One of Gy004Re, Gy010Re, Gy020Re, And Gy030Re.) GY004RE: Has R Made Medical Plans in Case of Future Serious Illness? GY010RE-GY021RE: R's Discussion of Health Care Plans And Preferences (With Anyone? Spouse? Kids? Whom? How Well Does Person Understand?). GY022RE-GY026RE: Legal Arrangements for Someone to Make Decisions About R's Medical Care (Any? Who Has, or Would Have, That Authority? Spouse? Kids?). GY030RE-GY038RE: R's Living Will or Advance Directive (Exists? Given to Spouse? to Kids? to Whom?). GY040RE-GY046RE: Inheritance Plans (Who Would Get R's Assets, or Spouse's? Does R Have Will? Etc.). GY060RE-GY064SP: Hopes/Plans for Last Days or Weeks of Life (Where Spent? Discontinue Treatments? for R And Spouse). GY065RE,GY066SP: How Strictly to Follow Wishes for End of Life Medical Care (for R And Spouse). GY070REC: At What Stage in the Terri Schiavo Case Did R Complete the WLS Phone Survey? This variable is set to "no" if none of the people mentioned in GY016RE, GY018RE, and GY020RE is a child. It is set to -2 "INAPPROPRIATE" if any of the three constituent variables is -1 or -3, or if this question doesn't apply because R is not in the End-of-Life sample or didn't discuss health care plans with anyone. In rare cases, respondents who according to our records are childless may nonetheless name a child. These cases are retained.

Each digit in this variable represents one child from R's child roster (and the digits are listed in the same order as the children in the roster). If R named a given child in GY016RE, that child's digit is set to 1; otherwise the child's digit is set to 2. E.g., a respondent with 3 children who named the eldest child and the youngest child would have this variable set to "121"; a respondent with 5 children who named none of them would have it set to "22222".
Please note that the child roster includes deceased children, as well as adopted and other non-biological children.
This variable is set to -2 INAP if R is not in sample (GY002RE == 0); if R did not discuss health care plans and preferences with anybody (GY016RE < 1); if R did not answer "2 CHILDCHILD-IN-LAW/GRANDCHILD" in response to any of wpa4a, wpa4d, wpa4g, wpb2, wpb3, or wpe9; or if R does not have any children listed in the child roster. It is set to -5 if the R dropped out of the interview before reaching this module ("-5 PARTIAL INTERVIEW").

Each digit in this variable represents one child from R's child roster (and the digits are listed in the same order as the children in the roster). If R named a given child in GY020RE, that child's digit is set to 1; otherwise the child's digit is set to 2. E.g., a respondent with 3 children who named the eldest child and the youngest child would have this variable set to "121"; a respondent with 5 children who named none of them would have it set to "22222".
Please note that the child roster includes deceased children, as well as adopted and other non-biological children.
This variable is set to -2 INAP if R is not in sample (GY002RE == 0); if R did not discuss health care plans and preferences with a third person (GY020RE < 1); if R did not answer "2 CHILDCHILD-IN-LAW/GRANDCHILD" in response to any of wpa4a, wpa4d, wpa4g, wpb2, wpb3, or wpe9; or if R does not have any children listed in the child roster. It is set to -5 if the R dropped out of the interview before reaching this module ("-5 PARTIAL INTERVIEW").

It is set to -2 "INAPPROPRIATE" if GY025RE is -1 "DON'T KNOW" or -3 "REFUSED", or if this question doesn't apply because R is not in the End-of-Life sample or didn't give medical authority to anyone. In rare cases, respondents who according to our records are childless may nonetheless name a child. These cases are retained.

This variable is set to 2 "NO" if none of the people mentioned in GY034RE through GY038RE is a child. It is set to -2 "INAPPROPRIATE" if any of the five constituent variables is -1 or -3, or if this question doesn't apply because R is not in the End-of-Life sample or R hasn't given the living will to anyone. In rare cases, respondents who according to our records are childless may nonetheless name a child. These cases are retained.
NOTE also that due to instrument error, some cases of -3 "refusal" in GY034RE through GY038RE may have been lost (and coded as -2). Thus, there may be a small number of cases in which this variable (GY033REC) will be set to 2 "NO" when it should in fact be set to -2 "INAPPROPRIATE".

Each digit in this variable represents one child from R's child roster (and the digits are listed in the same order as the children in the roster). If R named a given child in GY046RE, that child's digit is set to 1; otherwise the child's digit is set to 2. E.g., a respondent with 3 children who named the eldest child and the youngest child would have this variable set to "121"; a respondent with 5 children who named none of them would have it set to "22222".
Please note that the child roster includes deceased children, as well as adopted and other non-biological children.
This variable is set to -2 INAP if R is not in sample (GY002RE == 0); if no one is designated as executor of R's estate or would be responsible for distribution of R's estate (GY046RE < 1); if R did not answer "2 CHILDCHILD-IN-LAW/GRANDCHILD" in response to any of wpa4a, wpa4d, wpa4g, wpb2, wpb3, or wpe9; or if R does not have any children listed in the child roster. It is set to -5 if the R dropped out of the interview before reaching this module ("-5 PARTIAL INTERVIEW").
