Conversion into century months for 1992-94 follow-up studies

Century month = (YEAR * 12) + MONTH (1...12)

NOTE:  When only the year was obtained, century month is calculated
       using June: (year * 12) + 6. 

NOTE:  When only the season was obtained, century month is
       calculated using the mid-point of each season:
       (Summer = July; Fall = Oct; Winter = Jan; Spring = April)
       All dates for education were asked by season.
