A. “Calculating Disease-Based Medical Care Expenditure Indexes for Medicare Beneficiaries: A Comparison of Method and Data Choices,” by Anne E. Hall and Tina Highfill (w19720, December 2013, .pdf format, 23p.).

Abstract:

Disease-based medical care expenditure indexes are currently of interest to measurement economists. In this paper, using two data sources and two different methods for calculating disease-based expenditure indexes for the Medicare population, we establish some results that will help guide policymakers in choosing indexes for this population. First, we find that the two methods we examine (primary diagnosis and a regression-based method) produce the same results for the aggregate index and have a moderate level of agreement in which diseases contribute the most to growth in per capita health-care spending. Since the primary diagnosis method is preferable because of its transparency, this result implies that we may use the regression-based method when the data is not suitable for the primary diagnosis method without a great loss of accuracy. Second, we find that the two data sources, the Medicare Current Beneficiary Survey and the Medical Expenditure Panel Survey, produce very similar results in the aggregate but there is some evidence that they treat chronic illnesses differently. As the MCBS has a larger sample and more comprehensive coverage of Medicare beneficiaries than the MEPS, it seems that a regression-based expenditure index based on the MCBS is overall preferable for fee-for-service Medicare beneficiaries.

Abstract:

This paper uses data from the Health and Retirement Study to investigate the effects of Social Security’s Windfall Elimination Provision (WEP) and Government Pension Offset (GPO) provision on Social Security benefits received by individuals and households. WEP reduces the benefits of individuals who worked in jobs covered by Social Security and also worked in uncovered jobs where a pension was earned. WEP also reduces spouse benefits. GPO reduces spouse and survivor benefits for persons who worked in uncovered government employment where they also earned a pension.

Unlike previous studies, we take explicit account of pensions earned on jobs not covered by Social Security, a key determinant of the size of WEP and GPO adjustments. Also unlike previous studies, we focus on the household. This allows us to incorporate the full effects of WEP and GPO on spouse and survivor benefits, and to evaluate the effects of WEP and GPO on the assets accumulated by affected families.

Among our findings: About 3.5 percent of households are subject to either WEP or to GPO. The present value of their Social Security benefits is reduced by roughly one fifth. This amounts to five to six percent of the total wealth they accumulate before retirement. Households affected by both WEP and GPO lose about one third of their benefit. Limiting the reduction in the Social Security benefit to half the size of the pension from uncovered employment reduces the penalty from WEP for members of the original HRS cohort by about 60 percent.

C. “Time is Money: Life Cycle Rational Inertia and Delegation of Investment Management,” by Hugh H. Kim, Raimond Maurer, and Olivia S. Mitchell (w19732, December 2013, .pdf format, 35p.).

Abstract:

We investigate the theoretical impact of including two empirically-grounded insights in a dynamic life cycle portfolio choice model. The first is to recognize that investors incur opportunity costs in terms of current and future human capital accumulation when managing their own financial wealth, particularly if human capital is acquired via learning by doing. The second is that we incorporate age-varying efficiency patterns in financial decision making. Both enhancements produce inactivity in portfolio adjustment patterns consistent with empirical evidence. We also analyze individuals’ optimal choice between self-managing their wealth versus delegating the task to a financial advisor. Delegation proves most valuable to the young and the old. Our calibrated model quantifies welfare gains from including investment time and money costs, as well as delegation, in a life cycle setting.
